Updated ODDS!

Some may remember when I outline the odds of your getting a misfilled prescription (as well as dying as a direct result). You can refresh your memory HERE.

http://www.nsc.org/research/odds.aspx outlines the odds of your dying from everything imaginable. I could copy some over to this post, but that would insult your intelligence. I’ll just say this…you’re more likely to get struck by lightning taking a shit…


I think the worst misfil I’ve every had was when I got an insulin pen that was a humalog/novolog mix instead of just novolog probably wouldn’t have been lethal if I somehow managed to miss that (pretty damn hard considering its a completely different colour pen), but certainly not good.

Correct me if I am wrong, but what I hear (from the point you are making) is that too much hype is being put on DEATH CAUSED BY AN Rx ERROR. While this may be true, I still pound that message home to my CPhT’s as the worst case senario. Frankly, most of the CPhT’s I’ve worked with don’t have a clue as to the IMPORTANCE of ACCURACY within their job duties. Sloppiness = Inaccuracy = Incident Reports = Board Visits = Lawsuits.

True. I also contend that some of these “errors” are where the wrong doctor is put on an Rx label, wrong written date, wrong quantity dispensed (30 rather than 90), and wrong number of refills. While these are minute mistakes, I feel that calling them a drug error is somewhat of a misnomer. They in no way put the patient in danger, they are just bothersome.

It is NOT the wrong drug given to the wrong patient — which is the worst kind of TRUE drug error.

I’ve had a misfilled prescription. Someone put the wrong sticky labels on the bags, and I got given someone elses stuff… Theoretically, I suppose this is a drug error - wrong drug to wrong patient, but how dumb would I have to be not to notice that the drug name was not the same as my prescription? I just took it back and handed it in to the tech, and said, “Um, I’ wad supposed to get cocodamol, omeprazol, buscopan and tramadol. This looks like someone elses order.”

Now I always double-check before I leave the pharmacy.
